The Diabetes Program works to identify, provide education and health care services for the diabetic population which are the type 1, type 2, and gestational diabetic; and who utilize the I.H.S. and/or Tribal Health Department for their diabetes cares and management.

Mission, Vision & Goals:
​
Mission - The mission of the Cheyenne River Sioux Tribe Diabetes Program is to provide empowered learning and coping through an individualized journey involving support from family, peers, medical staff, and community resources in addition to a personal vision. This will enable patients to have hope and confidence to self-manage diabetes as part of their whole wellness.
​
Vision - The vision of the program is the prevention of diabetes complications through health promotion, patient education, and health care services. In addition providing self-management education to adults and children with type 1, LADA, and type 2 diabetes, as well as a pre-gestational and gestational population with diabetes.
​
Goals - Provide a participant-directed diabetes self-management education program. Additionally, to have a diabetic clinic in which Indian Health Service Diabetes Standards Care administered by physical and clinical staff.
