CHR Program's scope of work is to provide quality health care services and health promotion/disease prevention services to all eligible residents of the Cheyenne River Sioux Tribe as part of a comprehensive health care delivery system. The program addresses health care needs by providing community-oriented primary care services using Traditional Lakota health principles and community-based paraprofessional healthcare workers. The health services provided include but are not limited to health education, counseling, monitoring clients and communities, cased management, and coordination, emergency care, health promotional diseases prevention, transportation, language translation, non-emergency care, homemaker services, and environmental health monitoring. 

 

CHR POSITIVE PHARMACY CHANGES

                                                                                   

Due to the pandemic, CHRs will continue to pick up and deliver medications for all people residing on CRST to help keep people safe at home.

 

Pharmacy will batch medications for the CHR Program. What does this mean?  The CHR Program will have an identified space for medications to be stored so the CHRs can check several times throughout the day. But as the patient, you will need to call the pharmacy to authorize the CHR to pick it up for you. Please continue to call the CHR Program to request as well.

 

Morning requests-medications most likely to be ready that afternoon or next morning.

Afternoon requests- medications most likely to be ready the next morning or afternoon.

 

In outlying communities, all requests after 2:00 pm will be documented and delivered the next business day if ready. All in-town requests after 4:00 pm will be documented and delivered the next business day if ready. We kindly ask for you to remain at home so medications can be delivered. When you’re not home then medications are going to be delayed as CHRs have other clients to serve as well.

 

If narcotics are needed, please call Field Health @ (605) 964-0772, CHRs cannot handle narcotics. Medications will not be delivered if you’re under the influence of alcohol, medications will be returned to the pharmacy.

Commodity Supplemental Food Program 
 

What is Commodity Supplemental Food Program?

The Commodity Supplemental Food Program (CSFP) works to improve the health of low-income elderly persons at least 60 years of age by supplementing their diets with nutritious, 100 percent domestic USDA Foods.

CSFP is administered at the Federal level by the Food and Nutrition Service (FNS), an agency of the U.S. Department of Agriculture. Through CSFP, USDA distributes both food and administrative funds to participating States and Indian Tribal Organizations (ITOs). CSFP food packages are not intended to comprise a complete diet, but rather are good sources of the nutrients typically lacking in the diets of the target population.

Who is eligible for Commodity Supplemental Food Program?

In order to be eligible for this program, you must be:

·         Over 60 years of age

·         At or below 130 percent of Federal poverty income guidelines

States may require that participants be at nutritional risk, as determined by a physician or local agency staff. States may also require that an individual resides within the service area of the local agency at the time they apply for program benefits. However, States may not require that the individual resides within the area for any fixed period of time.
